Gadolinium Presence Within Cutaneous Sclerotic Bodies Confirmed by Laser Ablation Inductively Coupled Plasma Mass Spectrometry

Gadolinium-associated fibrotic plaques were first described by Cowper et al.1 The term nephrogenic systemic fibrosis (NSF) was introduced later to emphasize the potential for systemic involvement. Cutaneous sclerotic bodies (ossification with trapped elastic fibers) were described as a characteristic finding in patients with gadolinium exposure and systemic fibrosis.2- 4 In 2015, Gathings et al5 introduced the term gadolinium-associated plaques (GAP) to highlight the unique clinical presentation of small discrete plaques in the absence of systemic involvement or renal disease. The 2 patients they describe lacked the more common clinical findings of NSF and only 1 had renal disease. However, GAP is not universally recognized as an independent diagnostic entity. We describe the use of laser-assisted mass spectrometry to confirm deposition of gadolinium in sclerotic bodies of GAP.
