Association between appendectomy and Parkinson’s disease from the UK Biobank

Recently, the article by Konings et al systematically articulated the association between gastrointestinal syndrome and Parkinson’s disease (PD),1 which found that appendectomy reduced the risk of PD. According to Braak’s hypothesis2 that α-synuclein aggregation may begin in the gut and be transmitted to the brain via vagus nerve, appendectomy could remove α-synuclein and vagus nerve terminals in the appendix and may further reduce α-synuclein transmission and PD risk,3 which support Konings et al ’s conclusion. However, appendectomy may also lead to the release of harmful inflammatory pathogens into the adjacent intestinal milieu and the surgical procedure itself could potentially disrupt the integrity of the intestinal environment.4 Furthermore, given the robust and diverse microbiota in human appendix, such a procedure could disrupt the gut microbiome,5 6 potentially heightening the risk of PD which is also supported by previous studies.7 8 All these factors contribute to the ongoing debate regarding the association between appendectomy and PD. Hence, we investigated the impact of appendectomy on the development of PD using the comprehensive and rich participant information collected from the UK Biobank (UKB).
